Because of my employment, I’m on my feet a lot; I always get 6000 steps every day, but it’s usually 10,000-15,000.However, I have a BMI of 39. I am not sure what I am doing wrong here. It’s simply my stomach that is huge; it’s constantly swollen. I’m not coeliac, though; I’ve been tested for it.
I can’t exercise since I have a cardiac abnormality that causes shortness of breath and rapid heartbeat/palpitations when I do. Even walking causes problems for me, however as I already stated, I always do at least 6,000 steps per day. Can I lose weight without exercise?

Not a professional, but I believe your macros are incorrect. You might want to try a different meal with more protein. It’s also possible that your present diet is inflammatory for you, causing the bloating you’re describing, so altering things up wouldn’t harm.

As someone with PCOS, testosterone levels are insufficient to rule it out. If you have any other symptoms (hair loss, chin/chest/stomach/back hair, irregular periods), I would insist on getting an ultrasound. My testosterone levels are normal, but there are “lots of cysts on [my] ovaries”
With how much you’re walking, you should be good with activity, and if you’re actually in a deficit, you will lose weight. Continue to work with a doctor and, most likely, a dietician (not a nutritionist if you live in the United States). Those are quacks with no true education or qualification) and be more strict with your calorie counting. Because matter cannot be formed or destroyed, it is impossible to gain fat weight while on a calorie deficit.

Yes, it is possible. I have shed 40 pounds without exercising. I suffer chronic pain, and the rebound pain is not worth it. Do not skip meals. Eliminate the spaghetti soda and vodka. Replace it with electrolytes. Reduce your fat intake and boost your protein consumption. It will take much longer.
I would start with the bloating. Wheat causes me to feel bloated and makes it difficult to lose weight. I’m not coeliac, but I do have an intolerance. Maybe attempt a low fodmap elimination diet to identify your triggers.
